ALZHEIMER’S & DEMENTIA FAMILY SUPPORT

 Alzheimer’s & Dementia Family Support provides information & advice to families & friends caring for a loved one affected by Alzheimer’s or dementia.  We work throughout the districts of Medway & Swale.

WHAT CAN WE OFFER YOU?

Being diagnosed with a dementia related illness can be devastating.  Families & friends as well as the person diagnosed need information & support to help them with their challenging & changing needs.
Alzheimer’s & Dementia Family Support staff recognise the stresses, emotions & difficulties that a family have to cope with. 
Support is tailored to meet individual need.

· Practical & emotional support to adult family members 

· Support visits at home or a suitable place, if preferred

· Confidential telephone calls  

· Advice on services & benefits

· Help with accessing forms & submitting related claims  

· Contact local agencies which offer practical support

· Advocacy 

· Counselling

· Bereavement Counselling 

· Friendship Group

· Chair based exercises 

· Outings and events
